Ontario residents who take at least three prescription medications for chronic conditions are eligible for the program. Residency: The patient must be a resident of Ontario.
Number of Medications: The patient must be taking at least three prescription medications for a chronic condition.
Chronic Conditions: The medications must be prescribed for the treatment of chronic conditions, such as diabetes, hypertension, asthma, or any long-term health issues.
Ontario Health Insurance Plan (OHIP): The patient must have a valid OHIP card.
Additional specialized MedsCheck services have their own criteria:
MedsCheck for Diabetes: Available to patients diagnosed with type 1 or type 2 diabetes, regardless of the number of medications they are taking.
MedsCheck at Home: Available to patients who are unable to visit a pharmacy due to physical or mental health conditions, allowing a pharmacist to conduct the review at the patient’s home.
MedsCheck for Long-Term Care: Specialized for residents in long-term care homes.
